Activity Consent

Agreement to take part in activities organised by Chelsea FC Foundation.

I understand and hereby agree with the following statements:

• All the information I have provided is correct to the best of my knowledge
• No liability shall arise or occur against the Chelsea FC Foundation for any loss, damage or theft of personal property belonging to an individual, whilst participating in any activity,
• No liability shall arise or occur against the Chelsea FC Foundation for any death or injury occurring whilst participating in Chelsea activities except to the extent that such injury or death was the result of a negligent act or omission of the Chelsea FC Foundation, could have been reasonably avoided, and was foreseeable.

Photo Consent:

Photos and videos of you may be taken at the activity.The Foundation would like to use the Images to promote the work of the Foundation and grant Chelsea Football Club the right to do the same. This use may include print advertising, digital advertising and/or inclusion in presentations to partners, funders and other charitable organisations the Foundation works with or wishes to work with.
